COVID-19 Care Plan

As Florida continues to re-open, COVID-19 is continuously spreading throughout the United States. Due to the severe risk that the virus poses on the health of seniors, it is critical to have a care plan ready in case of a severe complication. You may be wondering, “what is a care plan?” A care plan is a summary that includes an individual’s health conditions and current treatments. The document should be easy to access, read, and express clearly the individual’s views, preferences, and needs. This resource can help reduce the number of hospitalizations and improve medical management, especially for seniors living in long-term care facilities.

Care plans can include:

  • A summary of your health conditions
  • Medications
  • Healthcare providers
  • Emergency contacts
  • End-of-life care options

Usually, people complete their care plans with their doctors, a family member, or even a home nurse aide. However, the summary should be carefully reviewed and approved by the individual who is going through the process of care.
